LifeSpring's Community Health Worker program aims to connect, educate and empower the families served at LifeSpring Pediatrics. Community Health Workers extend health care beyond our clinic walls, into the neighborhoods where our patients live, play, and attend school. They meet families where they are - in their homes, schools and churches. Community Health Workers provide:
Care Coordination: Community Health Worker's connect families to resources that support the whole family's wellness: from transportation to medical appointments to ESL & GED classes.
Health Education: We want to prevent disease before it starts, so our Community Health Workers focus on encouraging healthy behaviors such as healthy eating and physical activity. They meet with families one on one to set health goals and equip them with knowledge to take control of their health. They also organize group education classes for our community.
